- Type of product. E-cigarettes, vape pens, nicotine pouches, and cigars are all treated differently by some carriers. Be specific and honest on your application.
- Frequency of use. Daily vaping is viewed differently than occasional social use. Some carriers distinguish between habitual and infrequent users.
- Cotinine levels. Most applications that require a medical exam will include a urine or saliva test. Cotinine is the metabolite your body produces when it processes nicotine, and it stays detectable for several days. Even if you only vape occasionally, a positive cotinine result will trigger tobacco or nicotine classification.
- History of cigarette smoking. If you switched from cigarettes to vaping, carriers want to know when you quit smoking traditional tobacco. Many require 12 to 60 months of being cigarette free before they consider more favorable rates.
The underwriting process will also review your prescription history through database checks, your medical records if needed, and your motor vehicle records. For vaping specifically though, the nicotine test result and your answers about product type and frequency carry the most weight.
The Honesty Factor
We cannot stress this enough. Never hide your vaping from a life insurance application. Carriers check prescription databases, and cotinine testing will reveal nicotine use regardless of what you write on the application. If a company discovers you were dishonest, they can rescind your policy entirely during the contestability period (the first two years). That means your family could file a claim and receive nothing.

Instead, they rely on data from prescription databases, your medical history, and other electronic records.
There are tradeoffs to consider with no exam policies.
- Accelerated underwriting uses data algorithms to approve healthy applicants quickly. If you qualify, you may get competitive rates without a nicotine test. However, these programs are typically available only for lower coverage amounts and younger applicants.
- Simplified issue policies ask a limited set of health questions and skip the exam entirely. Rates are higher than fully underwritten policies, but the process is fast.
- Guaranteed issue policies require no health questions at all. Anyone who applies within the age range is accepted. Premiums are the highest of any option, and coverage amounts are usually capped at $25,000 to $50,000.
