Most people buy the first policy they find. They Google a name, fill out a form, and accept whatever number pops up on the screen. That approach almost always leaves money on the table. Two carriers can look at the exact same applicant and come back with rates that differ by 30% or more. The difference comes down to how each company weighs your health, your age, your history, and dozens of other details.

This guide breaks down exactly what determines your premium, how the underwriting process works, and how to position yourself for the lowest rate possible.

What Actually Affects Your Life Insurance Rate

Not every pricing factor carries the same weight. Some of them you can control. Others you cannot. Knowing the difference helps you focus your energy where it matters most.

Age is the single biggest factor. Rates climb roughly 8% to 10% for every year you wait. A 35 year old shopping today will almost always pay less than the same person shopping at 36. Procrastination has a real dollar cost.

Health comes next. Carriers look at your current conditions, how well those conditions are managed, and your overall health trajectory. A well controlled case of high blood pressure, for example, may barely affect your rate with some carriers while significantly increasing it with others.

Tobacco use can multiply your premium by two to four times. If you have quit within the last 12 months, most carriers still classify you as a tobacco user. However, some carriers reclassify you as a non-smoker after just 12 months, while others require three to five years. This is one area where shopping across carriers makes an enormous difference.

Build (your height and weight ratio) matters more than many people realize. Each carrier publishes its own build chart, and the guidelines vary widely. You might fall within acceptable limits at one company but land outside them at another.

Family history of heart disease or cancer before age 60 in an immediate family member can affect your rating class. Some carriers weigh this more heavily than others.

Rating Classes and Why They Matter

  • Preferred Plus (or Elite). This is the top tier. You need excellent health, a clean family history, and an ideal build. Rates here are the lowest available.

  • Standard Plus. Good overall health with a few manageable conditions. This is where many healthy applicants actually end up.
  • Standard. Average health. Controlled conditions, a build slightly outside ideal ranges, or a combination of smaller factors may place you in this category.
  • Table Ratings. If your health profile falls below Standard, carriers use a table rating system (Table 1 through Table 16). Each table adds approximately 25% to the Standard rate. Table 2, for example, means you pay 50% more than Standard.
  • Flat Extra. Some carriers add a temporary surcharge per $1,000 of coverage for specific risks. This flat extra often drops off after a set number of years.

Here is the part most people miss. Carriers do not all use the same criteria for each class. You might qualify as Standard Plus at one company and Preferred at another. That gap can translate into hundreds of dollars a year in savings.

How the Underwriting Process Works

Understanding what happens after you apply removes a lot of the mystery and helps you prepare.

The process typically begins with an application that asks about your medical history, lifestyle, occupation, and hobbies. From there, the carrier runs several background checks.

  • MIB Check. This database tracks your previous insurance applications. It does not contain medical records, but it flags conditions you disclosed on past applications.
  • Prescription Database (RxCheck). Carriers can see your prescription history going back several years. Be upfront about any medications because they will find them.
  • Motor Vehicle Records. A history of DUIs or reckless driving violations can affect your rate or your eligibility.
  • Medical Exam. Many traditional policies require a brief exam that includes blood work, a urine sample, and basic measurements. This typically takes 20 to 30 minutes and is done at your home or office at no cost to you.
  • Attending Physician Statements (APS). For more involved health histories, the carrier may request records directly from your doctor. This can add several weeks to the timeline.

A straightforward application with a medical exam usually takes three to six weeks from start to finish. More involved cases can stretch longer, which is why starting early matters.

How to Lock In the Best Rate

You have more control over your outcome than you might think. A few strategic moves can make a real difference.

Do not wait. Every birthday costs you money. If you are considering coverage, starting the process now protects you from future rate increases and any unexpected health changes.

Improve what you can. Losing weight, managing blood pressure, or lowering cholesterol before applying can bump you into a better rating class. Even modest improvements matter. If you recently quit tobacco, waiting until you pass the carrier’s non-smoker threshold can cut your premium dramatically.

Shop across multiple carriers. This is the single most effective strategy. Because each carrier uses its own underwriting guidelines, the same health profile can produce wildly different quotes. One company might penalize sleep apnea heavily while another treats it as a minor factor. One might be lenient on build while another is strict.

Work with an independent agent. An independent agent represents many different carriers rather than just one. That means they can match your specific health profile, budget, and coverage needs against a wide range of options. A captive agent (one who works for a single company) can only offer you that company’s products, even if a competitor would give you a better rate.

No Exam Policies vs. Traditional Underwriting

Accelerated underwriting skips the medical exam but still uses data (prescription history, MIB records, credit data) to evaluate your health. If the algorithm likes what it sees, you can get approved in days. Rates are often comparable to fully underwritten policies.

Simplified issue asks a limited set of health questions with no exam and no extensive data pull. Approval is fast, but premiums are higher because the carrier is taking on more risk.

Guaranteed issue requires no health questions at all. Anyone who applies within the age range gets approved. These policies carry the highest premiums and typically include a graded death benefit, meaning full coverage does not kick in for the first two to three years.

The tradeoff is straightforward. No exam policies offer speed and convenience. Fully underwritten policies usually offer lower rates for healthy applicants. For many people, accelerated underwriting hits the sweet spot of fast approval without a significant price penalty.
