Preferred Plus Life Insurance: How to Qualify for the Best Rates in 2026
Bottom Line. Preferred Plus life insurance is the highest rating class available, offering the lowest premiums to applicants in excellent health. Not every carrier defines “excellent” the same way, which means shopping across multiple insurers with an independent agent is the smartest move you can make.
What Preferred Plus Actually Means
Every life insurance carrier groups applicants into rating classes based on health and lifestyle. Preferred Plus (sometimes called “Super Preferred” or “Elite”) sits at the very top. If you earn this classification, you pay the absolute lowest premium that carrier offers for your age and coverage amount.
The difference in cost is significant. Someone rated Preferred Plus might pay 20% to 35% less than someone in the Standard category for the exact same policy. Over a 20 or 30 year term, that gap adds up to thousands of dollars.
Here is what makes this tricky. Each carrier sets its own guidelines for what qualifies as Preferred Plus. One insurer might allow a cholesterol reading up to 250, while another draws the line at 220. One might forgive a single moving violation, while another will not. That variation is exactly why working with an independent agency matters so much.
The Factors That Determine Your Rating Class
Your rate class comes down to several measurable factors. Some you can influence, and others you cannot.
Age has the single largest impact on what you pay. Premiums typically increase around 8% to 10% for every year you wait. A 35 year old in perfect health pays dramatically less than a 45 year old in the same condition. If you are considering coverage, applying sooner almost always works in your favor.
Overall health is the next major factor. Carriers look at blood pressure, cholesterol, blood sugar, and any current medications. For Preferred Plus, most insurers want to see readings well within normal ranges, not just borderline acceptable.
Tobacco use creates one of the biggest rate differences in the industry. Smokers and tobacco users typically pay two to four times more than nonsmokers. Most carriers require you to be tobacco free for at least 12 months, and many want three to five years of clean history for their best class.
Build (your height and weight ratio) plays a surprisingly large role. Every carrier maintains a build chart, and falling outside their acceptable range can drop you one or two rating classes regardless of how healthy you are otherwise.
Family medical history matters too. If a parent or sibling was diagnosed with heart disease or cancer before age 60, some carriers will exclude you from Preferred Plus. Others weigh this factor less heavily, which again highlights why shopping across carriers is so valuable.
Gender also affects pricing. Women statistically live longer and generally pay lower premiums than men at every rating class.
How Carriers Decide Your Class
The underwriting process is how an insurer gathers enough information to assign your rating. Understanding what happens behind the scenes can help you prepare.
It usually starts with your application, which asks detailed questions about your health history, lifestyle, occupation, and hobbies. From there, the carrier runs several database checks.
- MIB check. This reveals any previous life insurance applications and whether you disclosed conditions consistently.
- Prescription database (RxCheck). Carriers can see your pharmacy history, so every medication you have filled in recent years will show up.
- Motor vehicle records. Multiple speeding tickets, a DUI, or reckless driving charges can affect your rating class.
- Medical exam. For traditional underwriting, a paramedical professional collects blood and urine samples, checks your blood pressure, and records your height and weight.
- Attending physician statements. For more complex health histories, the carrier may request your medical records directly from your doctor.
The entire process typically takes two to six weeks for a fully underwritten policy. If your health profile is straightforward, some carriers offer accelerated underwriting that can skip the medical exam entirely and deliver a decision in days.
Realistic Expectations for Preferred Plus
Many applicants assume they will qualify for the best class only to receive a Preferred or Standard Plus offer instead. Here is a realistic look at what most carriers expect for their top tier.
- No tobacco use in the past three to five years
- Blood pressure at or below 130/85 without medication (some carriers allow well controlled medication)
- Total cholesterol generally under 240 (guidelines vary)
- BMI within the carrier’s build chart, typically under 28 to 30
- No significant family history of heart disease or cancer before age 60
- Clean driving record with no DUI history
- No hazardous hobbies like skydiving or private aviation (carrier dependent)
- No major medical conditions such as diabetes, heart disease, or sleep apnea
If you do not check every box, do not assume you are out of options. This is where the difference between carriers becomes your biggest advantage. One insurer’s “Preferred Plus” disqualification might be another insurer’s “no problem.”
No Exam Options and What They Cost
If you want coverage quickly or prefer to skip the medical exam, several paths exist.
Accelerated underwriting uses data from prescription databases, medical records, and other digital sources to make a decision without a physical exam. If you qualify, you can often get Preferred Plus rates with a much faster timeline. This is the best of both worlds for healthy applicants.
Simplified issue policies ask a limited set of health questions and require no exam. The tradeoff is higher premiums and lower coverage limits. These policies work well for people who need coverage fast or have mild health concerns that might complicate traditional underwriting.
Guaranteed issue policies ask no health questions at all. Anyone within the age range qualifies. However, premiums are the highest in the market, and most policies include a graded death benefit that limits payouts during the first two to three years.
The cost difference between fully underwritten coverage and no exam alternatives can be substantial. A healthy 40 year old male might pay $25 per month for a $500,000 fully underwritten term policy at Preferred Plus rates, while a simplified issue policy for the same amount (if even available at that level) could cost two to three times more.
How to Give Yourself the Best Shot at Top Rates
You have more control over your rating class than you might think. A few strategic moves can make a real difference.
Apply at the right time. If you have been improving your health through diet, exercise, or medication, give those changes time to show up in your lab work. Three to six months of consistent improvement can shift your numbers enough to qualify for a better class.
Get your labs checked first. Ask your doctor for a basic metabolic panel so you know where you stand before applying. Surprises during underwriting rarely work in your favor.
Be completely honest on your application. The MIB check and prescription database will reveal inconsistencies. Nondisclosure can lead to a declined application or, worse, a denied claim down the road.
Work with an independent agent who shops multiple carriers. This is the single most impactful thing you can do. A captive agent representing one company can only offer that company’s guidelines. An independent agent knows which carriers are more favorable for your specific health profile, your build, your family history, and your lifestyle.
