Why Your Rating Class Matters More Than You Think

Two people can apply for the exact same life insurance policy and receive wildly different prices. A 40 year old man in excellent health might pay half what another 40 year old man pays for identical coverage. The difference almost always comes down to one thing: the rating class the carrier assigns during underwriting.

If you qualify for Preferred Plus (sometimes called “Elite” or “Super Preferred” depending on the carrier), you are getting the absolute best price that company offers. Every step down from there means higher monthly premiums, sometimes significantly higher. That makes understanding how to qualify for this top tier one of the smartest moves you can make before applying.

What Underwriters Evaluate When Assigning Your Rate

Life insurance pricing is not random. Carriers use specific, measurable factors to place you into a rating class. Here is what matters most, roughly in order of impact.

Age. This is the single biggest driver of cost. Rates typically increase about 8% to 10% for every year of age. A 35 year old will almost always pay less than a 45 year old for the same coverage, even if both are in perfect health. This is why applying sooner rather than later can save thousands over the life of a policy.

Overall Health. Underwriters look at your current medical conditions and how well they are controlled. Blood pressure, cholesterol, blood sugar, and other markers all factor in. For Preferred Plus, most carriers want to see readings that fall within optimal ranges, not just “acceptable” ones.

Tobacco Use. Smokers and tobacco users pay two to four times more than nonsmokers. Most carriers require you to be completely tobacco free for at least 12 months to qualify for nonsmoker rates, and many want three to five years for Preferred Plus consideration.

Build (Height and Weight). Every carrier maintains internal height and weight guidelines. If your body mass index falls outside their acceptable range for the top class, you will be placed in a lower tier. These guidelines vary from one company to another, which is one reason shopping across multiple carriers matters so much.

Family History. If a parent or sibling was diagnosed with heart disease or cancer before age 60, some carriers will limit you to a lower rating class. Others are more lenient on family history. This is another area where the right carrier match makes a real difference.

Gender. Women statistically live longer and generally qualify for lower rates than men at the same age and health profile.

Rating Classes Explained

Most life insurance carriers use a tiered system of rating classes. While the names vary slightly between companies, the structure is fairly consistent.

  • Preferred Plus / Elite. The best rates available. Reserved for applicants in excellent health with no significant medical history, no tobacco use, a healthy build, and no concerning family history.
  • Preferred. Very good rates. You may have a minor, well controlled health issue or a family history factor that keeps you out of the top tier, but you are still healthier than average.
  • Standard Plus. Good rates for people with some health factors. Maybe your cholesterol is slightly elevated or your build is just outside Preferred guidelines.
  • Standard. Average rates for average health. This is the baseline most carriers use.
  • Table Ratings. For applicants with more significant health concerns, carriers may assign a table rating from 1 through 16. Each table adds roughly 25% to the Standard rate. A Table 4 rating, for example, means you pay Standard plus an additional 100%.
  • Flat Extra. Some situations call for a temporary surcharge, often expressed as an additional dollar amount per $1,000 of coverage. This is common for certain high risk hobbies or recent medical events and may be removed after a specified period.

One important thing we tell our clients regularly is that each carrier draws these lines differently. You might qualify as Standard Plus with one company and Preferred with another based on the exact same health profile. What Happens During Underwriting

When you apply for life insurance, the carrier gathers information from several sources to determine your rating class. Here is what to expect.

The process begins with an application that includes detailed health and lifestyle questions. From there, the carrier typically runs checks through the MIB (a database of past insurance applications), a prescription database called RxCheck, and your motor vehicle records. If the policy requires a medical exam, a paramedical professional will come to your home or office at no cost to you. They draw blood, collect a urine sample, and record your height, weight, and blood pressure. For more complicated health histories, the carrier may request your medical records (called an APS) directly from your doctor.

The entire process can take anywhere from a few days for accelerated underwriting programs to four to six weeks for traditional applications that require full medical records.

How to Position Yourself for the Best Rating

You cannot change your age or your family history, but there are real steps you can take to improve your chances of earning a Preferred Plus rating.

Get healthy before you apply. If your blood pressure or cholesterol is borderline, talk to your doctor about getting those numbers into optimal range before starting the underwriting process. Even a few months of consistent effort can make a difference.

Stop tobacco use well in advance. If you have recently quit, waiting until you hit the 12 month mark (or longer) before applying can move you from smoker rates to nonsmoker rates and potentially save you thousands of dollars.

Time your exam wisely. Schedule your medical exam for the morning after a good night of rest. Avoid heavy meals, alcohol, and intense exercise for 24 hours beforehand. These small choices can influence your blood pressure and lab results.

Shop across multiple carriers. This is where we see the biggest impact for our clients. A condition that one company penalizes heavily might be viewed much more favorably by another.

No Exam Options and When They Make Sense

Not every policy requires a medical exam. There are three main alternatives.

Accelerated underwriting uses data from prescription databases, credit history, and other electronic sources to make a decision without an exam. Many carriers now offer this for healthy applicants seeking coverage up to $1 million or more. If you qualify, you can sometimes get approved in days rather than weeks, and you may still earn Preferred Plus rates.

Simplified issue policies ask a limited set of health questions but skip the exam entirely. These are faster but typically cost more than fully underwritten policies and may cap coverage amounts.

Guaranteed issue policies require no health questions at all. Anyone who applies within the eligible age range is accepted. However, these come with the highest premiums and usually include a graded death benefit during the first two to three years.

The tradeoff is straightforward. The more information you provide to the carrier, the better your rate is likely to be, assuming you are in good health. If speed is your priority or you have health concerns that make traditional underwriting difficult, a no exam option might be the right fit.
