Each carrier uses its own formula to weigh your age, health history, build, occupation, and hobbies. One company might penalize a controlled thyroid condition while another barely blinks at it.

What Actually Affects Your Rate
Not every factor carries the same weight. Here is how insurers generally prioritize the variables that determine your premium.
Age is the single biggest driver. Rates climb roughly 8% to 10% for every year you wait. A healthy 30 year old might pay half of what a healthy 40 year old pays for the same policy. If you have been putting off coverage, every birthday that passes costs you real money.
Your current health matters more than you think. Carriers look at blood pressure, cholesterol, A1C levels, and any chronic conditions. The key word here is “control.” A well managed condition often qualifies for better rates than an uncontrolled one, even if the diagnosis is the same.
Tobacco use changes everything. Smokers typically pay two to four times more than nonsmokers for identical coverage. Most carriers require you to be tobacco free for at least 12 months before offering nonsmoker rates, though some require 24 or even 36 months.
Build (your height and weight ratio) plays a bigger role than many people expect. Each carrier publishes its own height and weight guidelines, and they vary significantly. You might fall outside the preferred range at one company but land squarely in it at another.
Family medical history can bump your rate up. If a parent or sibling was diagnosed with heart disease or cancer before age 60, some carriers will factor that into your classification.
Gender still affects pricing. Women statistically live longer and generally receive lower premiums than men of the same age and health profile.
Rating Classes and What They Mean for Your Wallet
When an underwriter reviews your application, they assign you a rating class. Think of it as a report card that determines your price tier.
- Preferred Plus (or Elite) is the best available rate. You need excellent health, no tobacco, a clean family history, and a build within tight guidelines. Only a small percentage of applicants land here.
- Preferred is still a very strong classification. Minor, well controlled health issues are usually acceptable. This is where many healthy applicants end up.
- Standard Plus sits between preferred and standard. You might have a couple of factors working against you but nothing severe.
- Standard reflects average health for your age. This is a perfectly normal classification and still provides affordable coverage.
- Table Ratings apply when health issues push you below standard. Tables run from 1 through 16, and each table adds roughly 25% to the standard rate. A Table 2 rating means you pay 50% more than standard.
- Flat Extras are additional charges per $1,000 of coverage, often used for temporary risks like a recent health event. These sometimes drop off after a set number of years.
Here is something most online calculators will never tell you. Different carriers draw the lines between these classes in different places. You might qualify as Standard Plus with one company and Preferred with another based on the exact same health profile. This is where shopping across multiple carriers becomes incredibly valuable.
What Happens During Underwriting
Understanding the process removes the mystery and helps you prepare.
It typically starts with an application. You will answer questions about your health, medications, lifestyle, driving record, and occupation. Be completely honest. Insurers cross reference your answers against several databases.
The MIB check reveals any previous insurance applications you have filed. If you reported a condition to one carrier five years ago, the new carrier will likely see a flag. The prescription database (sometimes called RxCheck) shows every medication filled in your name. Motor vehicle records reveal DUIs or reckless driving citations.
For traditionally underwritten policies, a medical exam follows. A paramedical professional takes your blood pressure, collects blood and urine samples, and records your height and weight. Results usually come back within a week or two.
For more involved health histories, the carrier may order your Attending Physician Statement (APS), which is a copy of your medical records. This can add a few weeks to the timeline but gives the underwriter a complete picture.
From start to finish, a fully underwritten policy typically takes three to six weeks. Complex cases may run longer.
No Exam Options and When They Make Sense
Not every policy requires a medical exam. Here are the main alternatives.
- Accelerated underwriting uses data from prescription databases, credit reports, and other electronic sources to make a decision quickly. If your profile is clean, you may get approved in days with rates comparable to a traditional exam policy.
- Simplified issue policies ask a limited set of health questions but skip the exam entirely. They cost more than fully underwritten policies and cap coverage amounts, but they work well for people who need coverage fast or want to avoid the exam process.
- Guaranteed issue policies ask no health questions at all. Anyone who applies within the age range gets approved. The tradeoff is significantly higher premiums and a graded death benefit that limits payouts during the first two to three years.
The general rule is straightforward. If you are in decent health and can wait a few weeks, traditional underwriting almost always delivers the lowest rate. If speed or health concerns make that impractical, no exam options fill an important gap.
How to Position Yourself for the Best Rate
You cannot change your age or family history, but there are factors within your control that can move the needle.
Apply sooner rather than later. Every year of delay costs you. If you are on the fence, remember that today is the youngest (and likely the healthiest) you will ever be.
