Substandard Life Insurance Guide for 2026

Bottom Line. A substandard life insurance rating means a carrier has approved your application but at a higher premium due to health or lifestyle factors. This does not end your search. Different carriers rate conditions differently, and shopping through an independent agent can save you thousands over the life of your policy.

What Does “Substandard” Actually Mean?

You applied for life insurance and got approved, but the offer came back with a “table rating.” That term can feel discouraging. The good news is that a substandard rating is still an approval. The carrier is willing to cover you. They just need to charge more to account for additional risk factors in your profile.

Every life insurance company groups applicants into rating classes after reviewing their health, lifestyle, and history. Most people have heard of Preferred or Standard ratings. Substandard (also called “table rated”) is the category below Standard, and it exists specifically so that people with real health concerns can still get meaningful coverage.

How Rating Classes Work

Life insurance carriers typically use a tiered system to classify applicants. Here is how the most common classes break down.

  • Preferred Plus or Elite is reserved for applicants in outstanding health with no significant family history concerns and an ideal build. These policyholders receive the lowest premiums available.
  • Preferred applies to applicants in very good health who may have a minor issue or two but nothing that raises serious concern.
  • Standard Plus covers people in good overall health with a few more factors on their record.
  • Standard reflects average health for someone at a given age. Most applicants land here or above.
  • Table Rated (Substandard) applies when an applicant has conditions or risk factors that fall outside the Standard range. Table ratings run from Table 1 through Table 16, and each table adds roughly 25% to the Standard premium.

That 25% increment matters. Someone rated at Table 2 would pay about 50% more than the Standard rate. Someone at Table 4 would pay about 100% more, effectively doubling the Standard premium. At the higher end, Table 8 means tripling the Standard cost.

Some carriers also use a “flat extra” approach instead of or alongside table ratings. A flat extra adds a fixed dollar amount per $1,000 of coverage, usually for a set number of years. This is common for recent health events where risk decreases over time.

Common Reasons for a Substandard Rating

Carriers look at several factors when deciding where you land. The biggest influences on your rating class include the following.

  • Age plays a major role. Rates increase roughly 8% to 10% for every year of age, and conditions that might earn a Standard rating at 35 could push toward substandard territory at 55.
  • Current health conditions matter significantly. Diabetes, heart disease, sleep apnea, and elevated blood pressure are common reasons for table ratings. How well you manage the condition often matters more than the diagnosis itself.
  • Tobacco use can multiply premiums by two to four times compared to a nonsmoker, and tobacco users with additional health concerns frequently receive substandard offers.
  • Build (height and weight) outside a carrier’s guidelines will affect your class. Every company has its own chart, and these vary more than most people realize.
  • Family history of heart disease or cancer before age 60 in immediate relatives can impact your rating even if your own health is strong.
  • Prescription history is checked through pharmacy databases. Carriers see what medications you have been prescribed, which often tells them more than the application alone.

Why Different Carriers Give Different Ratings

This is the single most important thing to understand about substandard life insurance. Not all carriers view the same condition the same way.

One company might rate a well controlled Type 2 diabetic at Table 4 while another offers Table 2 or even Standard for the same person with the same A1C numbers. One carrier might be strict on sleep apnea while another specializes in it and offers much better terms.

The process usually starts with an application that asks detailed health questions. The carrier will then check your MIB file (a database of previous insurance applications), your prescription history through RxCheck, and your motor vehicle records.

Many applications also require a medical exam, which typically includes blood work, a urine sample, and basic measurements. For more involved cases, the carrier may request your Attending Physician’s Statement (APS), which is a summary of your medical records from your doctor.

The entire process can take three to six weeks for substandard cases, sometimes longer if APS records are slow to arrive. Your agent should keep you updated at every step.

Alternatives to Table Rated Policies

If the table rating makes your premium unworkable, other paths exist.

  • Accelerated underwriting programs use data and algorithms instead of a medical exam. Some applicants who would receive a table rating through traditional underwriting can qualify for better classes through these newer systems.
  • Simplified issue policies ask a limited set of health questions and skip the exam entirely. Premiums are higher than fully underwritten Standard rates but may actually be lower than a high table rating on a traditional policy.
  • Guaranteed issue policies ask no health questions at all. These carry the highest premiums and usually include a graded death benefit in the first two to three years, but they guarantee acceptance regardless of health.

The right option depends on your specific situation, your coverage amount, and your budget. Sometimes a simplified issue policy for immediate coverage combined with a fully underwritten application (shopping for the best table rating) gives you both speed and long term savings.

How to Improve Your Chances

You cannot change your age or family history, but you can influence other factors before applying.

Improving control of existing conditions often makes a real difference. A diabetic who brings their A1C down from 8.5 to 7.0 could move from Table 4 to Table 2 or better with certain carriers. Losing weight to fall within a carrier’s build guidelines, quitting tobacco (most carriers require 12 months tobacco free for nonsmoker rates), and making sure all prescribed medications are current and consistent all help your case.

Timing matters too. Rates go up with age, so delaying too long while trying to improve health can backfire. Your agent can help you weigh the tradeoff between waiting for better health markers and locking in your current age.
