Aetna Medicare Advantage Review (2026–2027): Doctors & Costs

Bottom Line

Aetna Medicare Advantage may fit when the exact local plan covers your doctors and medicines with manageable medical costs. Check drug coverage carefully: an Advantage PPO without Part D cannot be paired with a standalone drug plan.

Get help comparing Medicare Advantage plans

Aetna Medicare Advantage is worth considering when a specific local plan includes your doctors, covers your prescriptions on workable terms and fits your budget for a difficult medical year. Start with those three tests. Aetna offers multiple Medicare products and asks for your ZIP code to identify local options; the company name alone does not establish coverage.

If you also own a business and are considering SBA financing, life insurance for an SBA loan is a separate planning question to discuss with your lender. It is unrelated to Medicare benefits or enrollment.

Start with the right Aetna product and coverage year

Medicare Advantage, also called Part C, provides your Part A hospital and Part B medical benefits through a private plan. Many plans include Part D prescriptions. Medicare Supplement insurance, or Medigap, instead helps pay certain Original Medicare cost sharing. A standalone Part D plan covers prescriptions separately. Aetna markets these as distinct products.

Record your county, intended coverage year, full plan name and contract/plan number. Download the Summary of Benefits, Evidence of Coverage, formulary and provider directory for that exact choice. Aetna’s document search distinguishes plans by location and product.

As you explore Medicare insurer reviews and comparisons, use the same doctors, medications and expected services to evaluate every candidate.

These published 2026 examples both list Montgomery County, Maryland, in their service areas. They illustrate different coverage choices; confirm availability for your address and enrollment date.

2026 feature Aetna Medicare Elite PPO, H5521-482 Aetna Medicare Eagle Giveback PPO, H5521-480
Monthly plan premium $46 $0
Medical deductible $1,200 for specified services $0
In-network primary care visit $0 $0
In-network specialist visit $60 $35
In-network medical out-of-pocket maximum $6,750 $6,750
Combined in/out-of-network medical maximum $7,750 $7,750
Part D drug coverage Included Not included

For someone who needs Part D, the last row may decide the comparison. You cannot add a standalone Medicare drug plan to a Medicare Advantage PPO without drug coverage; choose a PPO that includes Part D if you need it. An option without Part D calls for checking how any existing drug coverage will continue.

These are 2026 benefits, not a forecast of Aetna’s 2027 offerings. For January 2027 coverage, repeat the comparison using 2027 documents.

Will your doctors and hospitals work with the plan?

Search Aetna’s directory using the exact plan, then confirm participation with both Aetna and the provider’s office. Check the physician’s practice address, your preferred hospital, specialists, labs and any rehabilitation or equipment supplier you regularly use. Ask whether the office accepts new patients under that plan.

An HMO generally requires network care, with exceptions including emergency care, out-of-area urgent care and temporary out-of-area dialysis; HMO-POS plans can allow some additional outside care. PPOs generally cover out-of-network care at higher costs. Confirm that an outside provider will accept the plan before scheduling routine treatment.

Check prior authorization before choosing an Advantage plan, especially for planned surgery, hospital care, rehabilitation or expensive medications. Ask which services need approval and who submits the request. Referral requirements and authorization requirements are different questions; a plan that lets you see a specialist without a referral can still require approval for treatment.

For keeping your doctor after a network change, verify the departure date and ask about continued treatment arrangements before selecting replacement coverage. Aetna’s Elite policy describes assistance with ongoing care. Medicare evaluates special enrollment for significant network changes case by case.

Check every prescription at your actual pharmacy

Prepare each medication’s name, strength, quantity and refill frequency. Use Aetna’s drug tool to check the exact plan’s formulary, tier and deductible treatment. Look for prior authorization, step therapy and quantity limits. A drug appearing on the list does not answer all those questions.

Compare your regular pharmacy with available preferred pharmacies and mail order. Use Medicare’s comparison tool to estimate annual costs for your complete drug list, then check unresolved restrictions with the plan and your prescriber. No individualized drug-cost or network check has been performed for this review.

The federal out-of-pocket limit for covered Part D drugs is $2,100 in 2026 and $2,400 in 2027. It is separate from your Advantage plan’s medical maximum and does not make an excluded drug covered.

Compare the annual budget and the difficult year

A $0 plan premium still leaves the Part B premium obligation and applicable cost sharing. Check any verified Part B premium reduction separately. Compare hospital charges, outpatient surgery, imaging, therapy and specialist visits that matter to you. Medicare notes that plan costs depend on both the services used and whether you follow network rules.

Build two estimates:

  • Expected year: annual premiums, expected medical deductibles and copays, prescription costs and services outside coverage.
  • Heavy-use year: annual premiums plus the applicable medical maximum, a separate drug budget and costs excluded from the maximum.

The medical maximum is not a ceiling on every health expense. For example, Elite excludes plan premiums and Part D expenses from its medical maximum, and identifies additional excluded services in its benefits chart. Avoid counting deductibles twice when they already count toward that maximum.

Value dental benefits by the work you need

Aetna’s dental benefits and networks vary by plan. Before valuing an allowance, check limits on major dental work coverage in the actual documents: covered procedures, your share of the bill, annual benefit limits, frequency restrictions and dentist participation. Ask your dentist for procedure codes for planned crowns, dentures or other treatment, then request a benefit estimate.

A plan with useful preventive dental care may still leave a substantial bill for major work. Give extras weight only after doctors, drugs and medical costs pass your comparison.

Compare alternatives before committing

Aetna belongs on your shortlist when its local network and drug coverage fit, and its expected costs and medical maximum are manageable. If they do not, compare another local Advantage plan or Original Medicare with Medigap and standalone Part D. Original Medicare permits care from providers that take Medicare nationwide; Medigap cannot pay your Advantage plan’s copays.

When comparing Humana and UnitedHealthcare Medicare Advantage alongside Aetna, hold the county, coverage year, providers and drug list constant. Eliminate plans that fail a must-have need before comparing premiums or extras.

Confirm your enrollment route and any change notice

Medicare Advantage requires both Part A and Part B and residence in the plan’s service area. New beneficiaries may use an initial enrollment period; qualifying events can create special enrollment opportunities.

Fall enrollment runs October 15–December 7 for January 1 coverage. From January 1–March 31, someone already in Advantage may make one change: switch Advantage plans or return to Original Medicare, with an opportunity to add Part D. That change generally starts the month after the plan receives the request.

Annual Medicare open enrollment does not generally guarantee Medigap acceptance without medical underwriting. Federal Medigap open enrollment lasts six months, starting the first month you are at least 65 and enrolled in Part B. Outside that window, check guaranteed-issue protections and any additional state rights before leaving Advantage.

An ordinary premium or benefit change differs from plan termination. In deciding what to do when your Medicare Advantage plan ends, keep the notice and verify its reason, end date, replacement effective date and any Medigap rights. For a year-end contract nonrenewal, Medicare lists a special enrollment window from December 8 through the last day of February; other terminations have different deadlines. Arrange replacement coverage to begin when the old plan ends.

Prepare a comparison you can act on

Gather your ZIP code and county, desired start date, doctors and facilities, medication list, pharmacies and expected treatments. Compare two or three eligible plans using the same information. Save the documents and confirmation dates, resolve missing provider or drug answers, and check the enrollment effective date. If you have employer or retiree coverage, consult its benefits administrator before changing it.

Choose the plan whose verified access, prescription terms and total cost exposure fit your needs. Use Medicare’s plan comparison service or free State Health Insurance Assistance Program counseling to work through the remaining choices.
