How Does Preferred Plus Life Insurance Work?

Bottom Line. Preferred Plus life insurance is the highest rating class available, reserved for applicants in excellent health with clean family histories. Understanding how Preferred Plus life insurance works can help you position yourself for the lowest premiums any carrier offers, potentially saving thousands over the life of your policy.

The Best Rate Class Exists for a Reason

Not everyone pays the same price for life insurance. Two people of the same age and gender can receive dramatically different quotes depending on their health profile. The difference between a Standard rating and a Preferred Plus rating on a $500,000 policy can easily exceed $1,000 per year. That gap adds up fast over a 20 or 30 year term.

If you have ever wondered what it takes to earn the absolute best pricing, this is the rating class worth understanding.

What Preferred Plus Actually Means

Life insurance carriers group applicants into rating classes after reviewing their health, lifestyle, and personal history. Think of these classes as tiers on a pricing ladder.

  • Preferred Plus (sometimes called Elite or Super Preferred) is the top tier with the lowest premiums.
  • Preferred is the next level down, still excellent but with slightly higher rates.
  • Standard Plus falls in the middle, reflecting good overall health with a few minor concerns.
  • Standard represents average health and average pricing.
  • Table Ratings apply when health issues push someone below Standard. Each table (rated 1 through 16) adds roughly 25% to the Standard rate.

Preferred Plus is the gold standard. Carriers reserve it for applicants who present the lowest statistical risk. Earning this class means you checked every box the underwriter wanted to see.

The Factors That Determine Your Rating Class

Every carrier weighs these factors slightly differently, but the core criteria remain consistent across the industry. Here is what underwriters evaluate, ranked roughly by how much each one affects your final classification.

Age. Life insurance rates increase approximately 8 to 10 percent for every year you wait to apply. A healthy 30 year old will pay significantly less than an equally healthy 45 year old. Locking in a Preferred Plus rate while young creates enormous long term savings.

Overall Health. This is the big one. Preferred Plus applicants typically need pristine blood work, healthy cholesterol and blood pressure readings, and no active medical conditions. Even well controlled conditions like mild high blood pressure may bump you down to Preferred at some carriers, while others may still approve you at the top tier.

Tobacco and Nicotine Use. Smokers and tobacco users pay two to four times more than nonsmokers. For Preferred Plus, most carriers require that you have been completely tobacco free for at least three to five years. Some carriers include vaping and nicotine patches in this restriction while others do not.

Build (Height and Weight). Every carrier maintains a build chart that maps acceptable weight ranges to your height. Falling outside those guidelines, even by a few pounds, can move you from Preferred Plus to Preferred. These charts vary meaningfully between carriers, which is one reason shopping matters so much.

Family Medical History. Underwriters look at whether your parents or siblings developed heart disease, cancer, or diabetes before age 60. A clean family history strengthens your case for Preferred Plus. A family history of early onset disease does not automatically disqualify you, but it may limit which carriers offer you the top class.

Gender. Women statistically live longer than men, so female applicants generally receive lower rates at every classification level, including Preferred Plus.

Driving Record and Lifestyle. Multiple moving violations, a DUI within the past several years, or high risk hobbies like skydiving or private aviation can affect your eligibility. Carriers want to see a stable, low risk lifestyle.

What the Underwriting Process Looks Like

When you apply for a policy at the Preferred Plus level, the carrier digs deeper than you might expect. The process typically includes several steps.

  • The application itself. You will answer detailed questions about your medical history, medications, occupation, and lifestyle.
  • MIB check. The Medical Information Bureau maintains a database of previous insurance applications. Underwriters review this to confirm consistency with what you reported.
  • Prescription database review (RxCheck). Carriers can see your prescription history going back several years. This reveals conditions you may not have mentioned or medications that suggest underlying health concerns.
  • Motor vehicle records. Your driving history is checked for patterns that suggest risk.
  • Medical exam. For Preferred Plus on larger policies, most carriers require a paramedical exam that includes blood draws, urine samples, blood pressure checks, and height and weight measurements.
  • Attending Physician Statements (APS). If the underwriter spots something that needs clarification, they may request your medical records directly from your doctor. This can add time to the process.

The entire underwriting timeline typically runs two to six weeks depending on how quickly records are gathered.

Why the Same Person Gets Different Offers

This is the part most people miss. Each insurance carrier builds its own underwriting guidelines. One company might be strict about build charts but lenient on family history. Another might penalize a single elevated cholesterol reading while a competitor ignores it if every other marker looks strong.

That means you could qualify for Preferred Plus at one carrier and only Preferred or Standard Plus at another. The financial difference between those outcomes is significant.

This is exactly why working with an independent agency matters. A captive agent who represents a single company can only offer you that one carrier’s decision. An independent agent shops your profile across many different carriers to find the one most likely to offer you the best classification.

Our Approach to Finding Your Best Rate

Insurance By Heroes was founded by a former first responder and military spouse, and every member of our team comes from a background in public service. That service first mindset shapes how we work with clients every single day. We treat the process of protecting your family the way we once approached protecting our communities.

Because we are an independent agency, we are not locked into any single carrier. When we help clients pursue a Preferred Plus rating, we know which carriers are most favorable for their specific health profile, build, and history. We compare options across many different carriers to find the right match. That independent advantage means you are not leaving money on the table by settling for whatever one company decides.

Whether you are a teacher, a small business owner, a fellow first responder, or anyone else who wants to protect the people who depend on you, our team applies the same level of care and attention to your case.

How to Improve Your Chances of Qualifying

You cannot change your age or family history, but several factors are within your control.

  • Get healthier before applying. Losing weight, improving your cholesterol through diet or medication, and maintaining healthy blood pressure all make a measurable difference.
  • Quit tobacco products early. If you recently quit, waiting until you clear the three to five year window can save you thousands in premiums.
  • Clean up your driving record. Let any violations age off your record before applying if possible.
  • Time your application strategically. If you know you will be in better health in six months, it may be worth the short wait. But remember that rates increase with age, so do not delay indefinitely.
  • Be completely honest on your application. Underwriters will uncover inconsistencies through prescription databases, medical records, and the MIB. Honesty builds credibility and avoids complications later.

No Exam Options and the Tradeoff

Some carriers offer accelerated underwriting programs that use data (prescription history, credit information, and digital health records) instead of a traditional medical exam. If your profile is strong, you may receive a Preferred Plus decision in days rather than weeks.

Simplified issue and guaranteed issue policies skip most of the health screening entirely. These products serve an important purpose for people who cannot qualify through traditional underwriting, but they come with higher premiums and lower coverage limits. If your goal is to earn Preferred Plus rates, the traditional underwriting path (including the medical exam) almost always produces the best pricing.

The general rule is straightforward. The more information you are willing to share with the carrier, the better your rate is likely to be.
