Life Insurance for Smokers: Your 2026 Guide to Better Rates

Bottom Line. Life insurance for smokers costs significantly more than coverage for nonsmokers, but the price gap between carriers can be enormous. By understanding how each company classifies tobacco use and shopping across multiple insurers, smokers can often cut their premiums dramatically without sacrificing coverage.

If you use tobacco in any form, you have probably already seen the sticker shock. Quotes that seem reasonable for a nonsmoker suddenly double, triple, or even quadruple once you check the “yes” box next to the tobacco question. That price jump is real, but it does not have to be the end of the story. The difference between the most expensive and least expensive carrier for a smoker can be hundreds of dollars per year for the same amount of coverage.

Why Smokers Pay More (and How Much More)

Insurance companies price policies based on risk, and tobacco use remains one of the strongest predictors of early mortality in actuarial data. On average, smokers pay two to four times more than nonsmokers for the same term life policy. A healthy 40 year old nonsmoker might pay $30 per month for $500,000 of 20 year term coverage. That same person as a classified smoker could pay $90 to $130 per month depending on the carrier.

That range matters. The gap between $90 and $130 is almost $500 per year, and both quotes are for the exact same coverage amount from financially strong companies. This is why shopping matters more for smokers than almost any other group of applicants.

What Counts as “Tobacco Use” in Underwriting

Here is where things get interesting, because every carrier defines tobacco use a little differently. Most companies will classify you as a smoker or tobacco user if you have used any of the following within the past 12 months.

  • Cigarettes
  • Cigars (even occasional)
  • Chewing tobacco or snuff
  • Nicotine patches or gum
  • Vaping or e cigarettes
  • Nicotine pouches

However, not all carriers treat every product the same way. Some companies offer a separate “cigar only” rate class for people who smoke a handful of cigars per year but do not use cigarettes. Others may offer preferred smoker rates for people who vape but test negative for cotinine (a nicotine byproduct) at lower thresholds. A few carriers even distinguish between marijuana use and nicotine use, with some treating occasional marijuana users as nonsmokers if no nicotine is present.

Rating Classes for Tobacco Users

Not every carrier offers this class, but the savings over standard smoker rates can be significant.
  • Standard Smoker (or Standard Tobacco). This is the default classification for most tobacco users. If you have some health issues on top of tobacco use, you will likely land here.
  • Table Rated Smoker. If you have serious health concerns in addition to tobacco use, a carrier may add a table rating on top of smoker rates. Each “table” adds roughly 25% to the standard smoker premium. A Table 2 rating means you are paying 50% more than the standard smoker base.

The key takeaway is that different carriers draw the line between these classes differently. One carrier’s standard smoker might be another carrier’s preferred smoker, and that distinction alone can save you 15 to 25% on your annual premium.

The Underwriting Process for Smokers

When you apply for a traditionally underwritten life insurance policy, the carrier will verify your tobacco status through several methods.

  • The application itself. You will be asked directly about tobacco and nicotine use, usually covering the past 12 to 60 months depending on the carrier.
  • Medical exam. If your policy requires a paramedical exam, your blood and urine will be tested for cotinine. This chemical stays in your system for several days after nicotine use, making it very difficult to hide recent tobacco consumption.
  • Prescription database checks. If you have been prescribed nicotine cessation products like Chantix or nicotine replacement therapy, this will appear in your records.
  • MIB records. If you previously applied for insurance and disclosed tobacco use, that information may be stored in the Medical Information Bureau database.

Honesty on the application is not just an ethical obligation. It protects your family. If a carrier discovers undisclosed tobacco use after you pass away, they could deny the death benefit entirely during the contestability period (typically the first two years of the policy). The financial consequences of that denial would fall directly on the people you were trying to protect.

How to Get the Best Rates as a Smoker

Even within the smoker category, there are real strategies that can lower your premiums.

Quit and wait. Most carriers will reclassify you as a nonsmoker after 12 months of being tobacco free. Some require 24 months, and a few require as many as 60 months. If you recently quit, applying with a carrier that has a shorter lookback period can save you thousands over the life of a policy. Many policies also allow you to request reclassification after you have been tobacco free for the required period, meaning you can apply now at smoker rates and then reduce your premium later without buying a new policy.

Get healthy in other areas. Since tobacco use already pushes you into a higher rate class, any additional risk factors compound the cost. Losing weight, managing blood pressure, and controlling cholesterol can all help you land in a better smoker subclass rather than taking table ratings on top of already elevated premiums.

Shop across many carriers. This cannot be overstated. The variation in smoker pricing across the life insurance market is wider than almost any other rating factor. One carrier might charge a 40 year old male smoker $110 per month for $500,000 of 20 year term coverage while another charges $85 for the same policy. Multiply that $25 monthly difference over 20 years and you are looking at $6,000 in savings.

No Exam Options for Smokers

If you want coverage quickly or prefer to skip the medical exam, several options exist.

Accelerated underwriting programs use data from prescription databases, motor vehicle records, and other electronic sources to make a decision without a physical exam. Some of these programs are available to smokers, though the coverage limits may be lower than what a fully underwritten policy would offer.

Simplified issue policies ask a limited set of health questions but require no exam and no lab work. These policies cost more per dollar of coverage than fully underwritten policies, but they can be issued within days rather than weeks.

Guaranteed issue policies accept everyone regardless of health or tobacco status. There are no health questions at all. The tradeoff is that premiums are the highest of any product type, coverage limits are typically capped at $25,000 to $50,000, and most policies include a graded death benefit that limits payouts during the first two to three years.

For most smokers in reasonable health, a fully underwritten policy will offer the best value. But if health issues make traditional underwriting unlikely to produce a favorable result, simplified or guaranteed issue products can still put meaningful protection in place for your family.
