Understanding Preferred Life Insurance and How It Saves You Money

Bottom Line. Understanding preferred life insurance rating classes can save you thousands over the life of your policy. Carriers assign different rate tiers based on your health, habits, and history, and qualifying for a preferred class means significantly lower premiums for the same coverage amount.

Two people the same age, applying for the same coverage amount, can receive wildly different premium quotes. The difference often comes down to one thing: the rating class assigned during underwriting. If you understand how carriers evaluate applicants and what separates one rating tier from another, you put yourself in a much stronger position to lock in the best possible price.

What Is a Preferred Rating Class?

Every life insurance carrier groups applicants into rating classes based on risk. Think of it as a grading system. The healthier and lower risk you appear, the better your “grade” and the lower your premium.

Most carriers use some version of these tiers.

  • Preferred Plus (or Elite) is reserved for applicants in excellent health with no significant medical history, no tobacco use, and a clean family health background. This class gets the absolute lowest rates.
  • Preferred is for those in very good health who may have a minor, well controlled condition. Rates here are still well below average.
  • Standard Plus sits between preferred and standard. Applicants might have a couple of manageable health factors but nothing alarming.
  • Standard reflects average health for someone of your age and gender. This is the baseline rate most carriers quote initially.
  • Table Ratings (Substandard) apply when health conditions or risk factors push you beyond standard. Tables typically range from 1 through 16, and each table adds roughly 25% to the standard premium.
  • Flat Extras are an additional charge per $1,000 of coverage, often applied temporarily for specific risks like a recent health event or hazardous occupation.

Here is the part most people miss. Each carrier builds its own underwriting guidelines. One company’s preferred class might accept a condition that another company would rate as standard. That variation is exactly why shopping across multiple carriers matters so much.

The Factors That Determine Your Rating Class

Several factors work together to determine where you land. Some you can control, and some you cannot.

Age is the single biggest pricing factor. Rates typically climb about 8% to 10% for every year you wait. A 35 year old will pay meaningfully less than a 40 year old for identical coverage, all else being equal. This is why acting sooner rather than later almost always works in your favor.

Overall health plays a major role. Carriers look at blood pressure, cholesterol, blood sugar levels, and body mass index. Well controlled conditions often qualify for preferred or standard plus classes, while uncontrolled conditions push ratings higher.

Tobacco use creates one of the largest rate differences in life insurance. Smokers typically pay two to four times more than nonsmokers. Most carriers require you to be tobacco free for at least 12 months to qualify for nonsmoker rates, though some require longer.

Build (height and weight) is evaluated against each carrier’s internal chart. Falling outside the acceptable range for your height can bump you into a lower rating class, even if everything else looks great.

Family medical history matters more than many applicants expect. If a parent or sibling developed heart disease or cancer before age 60, some carriers will limit you to standard class regardless of your personal health.

Gender also affects pricing. Women statistically live longer and generally receive lower premiums than men at the same age and health level.

What Happens During Underwriting

Understanding the process itself removes a lot of the mystery. When you apply for a traditionally underwritten policy, the carrier gathers information from several sources.

The application includes detailed health and lifestyle questions. From there, the carrier runs a check through the MIB database, which tracks your history of applying for insurance. They also pull prescription records through a database commonly known as RxCheck, and they review your motor vehicle record.

Many policies also require a medical exam, which typically involves a blood draw, urine sample, blood pressure reading, and basic measurements. For more complex health situations, the carrier may request your actual medical records from your doctors, known as an APS (Attending Physician’s Statement).

The entire process usually takes two to six weeks for traditional underwriting, though timelines vary by carrier and complexity.

No Exam Options and the Tradeoff

If speed is your priority, no exam options exist. These fall into a few categories.

Accelerated underwriting uses data from prescription databases, public records, and sometimes even credit history to make a decision without a medical exam. Healthy applicants can sometimes get preferred rates this way, and approval can happen in days rather than weeks.

Simplified issue policies ask a limited set of health questions but skip the exam entirely. Coverage amounts are usually capped, and rates run higher than fully underwritten policies.

Guaranteed issue policies ask no health questions at all. Anyone who meets the age requirements gets approved. However, premiums are the highest of any option, and coverage amounts are limited. These policies also typically include a graded death benefit, meaning full coverage does not kick in for the first two to three years.

The tradeoff is straightforward. The less information you provide, the more the carrier charges to offset the unknown risk. If you are reasonably healthy and can wait a few weeks, full underwriting almost always delivers a better rate.

How to Position Yourself for Preferred Rates

You have more influence over your rating class than you might think. Here are practical steps that can make a real difference.

  • Apply at the right time. If you are actively losing weight, improving your blood pressure, or about to hit a tobacco free milestone, waiting a few months could shift your rating class.
  • Get your numbers checked first. Knowing your blood pressure, cholesterol, and A1C levels before applying helps set realistic expectations and avoids surprises.
  • Be completely honest on your application. Carriers verify everything. Omissions or inconsistencies can delay your approval or lead to a denial.
