What Is Preferred Plus Life Insurance?

If you have ever compared life insurance quotes, you may have noticed that rates vary dramatically from one person to the next. Even two people the same age and gender can pay very different premiums. The reason comes down to rating classes, and Preferred Plus sits at the very top.

A Preferred Plus rating (sometimes called “Elite” or “Super Preferred” depending on the carrier) means the insurance company considers you their lowest risk. You are in outstanding health, you have a clean family medical history, and your lifestyle presents minimal concern. In return, the carrier rewards you with the most affordable rates they offer.

This matters more than most people realize. The difference between a Preferred Plus rating and a Standard rating on a $500,000 policy can easily amount to hundreds of dollars per year. Over a 20 or 30 year term, that gap adds up to thousands of dollars in savings.

Preferred Plus Life Insurance Explained: Rating Classes and What They Mean

Every life insurance carrier uses a tiered system to classify applicants. While the exact names and criteria vary, most carriers follow a structure similar to this.

  • Preferred Plus / Elite. The best possible rating. Reserved for applicants with excellent health, no tobacco use, a healthy build, and no significant family history of heart disease or cancer before age 60.
  • Preferred. Very good health overall. Minor, well controlled conditions may be acceptable. Still offers very competitive pricing.
  • Standard Plus. Good health with a few more factors present. Rates are higher than Preferred but still reasonable.
  • Standard. Average health. This is the baseline rating class, and many Americans fall here.
  • Table Ratings (Substandard). For applicants with more significant health concerns. Table ratings run from Table 1 through Table 16, and each table adds roughly 25% to the Standard rate.
  • Flat Extra. An additional charge per $1,000 of coverage, often applied temporarily for specific risks.

Here is the part that surprises many of our clients. Each carrier draws its own lines between these classes. One carrier’s Preferred Plus criteria might be stricter or more lenient than another’s. A slightly elevated cholesterol reading might disqualify you from the top tier at one company while another carrier considers it perfectly acceptable. How Carriers Decide Your Rating Class

When you apply for life insurance, the carrier evaluates several factors to determine where you land. Understanding these factors gives you a realistic picture of what to expect.

Age is the single biggest driver of life insurance pricing. Rates typically increase about 8 to 10 percent for every year you wait. A healthy 35 year old will pay significantly less than a healthy 45 year old for the same coverage.

Overall health plays the next largest role. Carriers look at blood pressure, cholesterol, blood sugar, and any existing medical conditions. Well controlled numbers work in your favor. Uncontrolled or undiagnosed conditions do not.

Tobacco use creates one of the largest price gaps in life insurance. Smokers typically pay two to four times more than nonsmokers. Most carriers require you to be tobacco free for at least 12 months to qualify for nonsmoker rates, though some require longer.

Build (height and weight) factors into every carrier’s underwriting guidelines. Each company publishes its own acceptable ranges. If your build falls outside one carrier’s guidelines, another carrier may still offer favorable terms.

Family medical history also matters. If a parent or sibling was diagnosed with heart disease or cancer before age 60, some carriers will restrict you from their top rating class. Others weigh this factor differently.

Gender affects pricing as well. Women generally pay less than men at any given age because of longer average life expectancy.

The Underwriting Process: What to Expect

Once you submit an application, the carrier begins gathering information. The process typically includes several steps.

  • Application questions cover your health history, lifestyle, occupation, hobbies, and tobacco use.
  • MIB check reviews your history with other insurance applications.
  • Prescription database review (RxCheck) shows any medications you have been prescribed in recent years.
  • Motor vehicle records flag any DUI history or major violations.
  • Medical exam (if required) includes blood work, urine sample, blood pressure, and height and weight measurements.
  • Attending Physician Statements (APS) may be requested for more complex medical histories.

Get your numbers in check. If your blood pressure or cholesterol is borderline, talk to your doctor about strategies to improve them before applying. Even small improvements can shift you into a better rating class.

Be honest on your application. Carriers verify your information through medical records, prescription databases, and lab results. Inconsistencies raise red flags and can delay or complicate your approval.
